International Index of Erectile Function Questionnaire

This is a validated questionnaire used by doctors. Each question has several responses, and you should choose the one that best describes your own situation.

Question 1 of 6. Over the past 4 weeks, how often were you able to get an erection during sexual activity?
Question 2 of 6. Over the past 4 weeks, when you had erections with sexual stimulation, how often were your erections hard enough for penetration (entering your partner)?
Question 3 of 6. Over the past 4 weeks, when you attempted intercourse, how often were you able to penetrate (enter) your partner?
Question 4 of 6. Over the past 4 weeks, during sexual intercourse, how often were you able to maintain your erections after you had penetrated (entered) your partner?
Question 5 of 6. Over the past 4 weeks, during sexual intercourse, how difficult was it to maintain your erection to completion of intercourse?
Question 6 of 6. Over the past 4 weeks, how do you rate your confidence that you could get and keep an erection?
